When I was a med student I spent a day with the fire department in Phoenix AZ in July, we had to rescue a heat stoked hiker off a moderately chunky trail. With four of us using one of these stretchers, it worked like a charm. I’d never seen one before either, and can’t really think of any other options for technical terrain other than a helicopter (which happens a lot here, too)

I’ve unfortunately been there to see a buddy carried out on one of these. (alive, injured but still with us today) And I’ve practiced with a normal non-wheeled basket, briefly.
The single-wheel ones are pretty great, and WAY easier than trying to just carry a basket with no wheels, which sucks and has plenty chance of injury (especially ankle twists) for those trying to maneuver the basket over rough terrain.
The only real downside to these is you can’t just set them down to rest or if the attendants need to reposition.

Anyone else triggered by the cable routing for the disk brake here? In the PNW, that’s gonna get tangled up in branches and lead to injuries: needs some zip ties!
This is a great design. The one wheel is ideal for singletrack, off-camber terrain and tight maneuvers. Might be improved with a suspension fork (comfort) and downrigger stabilizers (legs you could fold down to support the load while providing assistance to the victim)?

I work in EMS and I mean that’s kind of cool, but we already have lots of tools and different stretcher options for extricating people over rough terrain that seem better than this. I also wouldn’t want to be a patient with an open leg fracture rolling over rough terrain.
