    You will most likely need a 6mm and 4mm Alan wrenches. But take off the top cap and loosen the stem pinch bolts, and finally the fork should legit slip right out.

    for removal you don’t need anything specific. just undo those 3 (2 on the stem, 1 on the cap) hex bolts, and the fork should come out. 

    be careful in doing so:  depending on the style of the headset, tiny balls could come out. I’d recommend taking out the upper parts of the headset first, and then taking care of the lower parts.

    what you’re looking at is headset refurbishing/overhaul – it’s not granted that the whole thing is in decent condition… i wouldn’t bet on it, seeing the amount of rust on the frame. also, the creaking.
