Could be that your star nut or steerer bung has come loose and spins together with the top cap bolt

What happens if you pull up on the stem while turning the bolt? Can you look through the steertube from the bottom and see if the retainer is turning with the bolt?

If it just spins, then there should be nothing besides the headset side clamp bolts holding your headset onto your bicycle. You should be able to lift the stem off, if it gets stuck. You should be able to continue to unscrew the bolt. Either way, there is nothing stopping your headset from coming off your bike. Good luck 👍🏻

Sounds like one part of the star fangled nut has separated from the other part, which does happen.

Drill the head of the preload bolt off. Take your time so you don’t screw the top cap itself. The threaded portion of the star nut has come loose and is spinning freely. You might get away with prying up on the top cap but that will Mar up the stem.
