    Hard to know exactly – the chain is probably worn (stretched), but the chainring also looks work (the big one).

    It may be that you change the chain and it doesn’t get better because the chainring is also worn.

    If the chain is falling down onto the smaller chainring, then it could be your derailleur needs adjusting too… but the chain obviously has some issues given the picture.

  2. Measure it with a ruler. 24 links (center to center) should be 12-inches. If you see something like 12 1/8 inches (or more) than your chain is stretched (which is really wear on the rollers).

    Sweet Jesus. Every part of that drivetrain, including the chainrings, will need to be replaced.
