Chain tug
It is a chain tensioner. You don’t need it, the nuts that hold the wheels on will prevent it from moving around.

Chain tensioner, most dont need it but if you race or just have incredibly strong legs, you could pull your wheel out of alignment if you dont have it. But thats rare in terms of leisure riding.
Most people have let you know that you don’t need it. If you choose to use one, make sure you install it on the other side of the frame from what is shown in your picture. It’s designed to work better that way.
