It worked itself loose over the life of the chain. I never noticed it riding….don’t really use that end of the cassette but was putting a new chain on and realized that I should keep an eye on this more.
sram gx axs b tension was waaaaaay off.

You sure your chain is long enough?
Chain is too short.
seems more of a setup/installation error than anything else.
