My rigid singlespeed developed a headset creak on my last ride. Imagine my surprise when, upon loosening the stem quill bolt, the top half of the headset came off with the stem. The tool marks on the broken stub are from the extractor I used to get the stub off the stem.

I’ve not seen a steel steerer break like that. But then again, I haven’t seen everything.
