I just bought a generic crank extractor that didn't specify any sizes so I assumed it was a universal fitting, but the inner thread is too narrow. Am I doing something wrong or did I get the wrong tool?

Yes, there are extractors with bigger diameter additional pads for such axles.
Tho I used a few times a random nut that fits in there.
Use a 5p if you’re in the uk to convert a square taper puller to isis ( like you have ).
Basically any coin / thin metal disc that will cover the axle and not the splines of the crank 😊
