Sometimes the dishing is perfect, sometimes it’s out of dish. It depends on the axle position in which the wheel is installed. I first thought the axle was bent, but it’s not. And everything inside the hub is installed nice and tight. So I can’t figure out why the dishing keeps changing depending on the axle position! Just crooked hub? Thank you for help!

The dish is probably slightly out, so if you reverse the wheel in the dropout it dishes in a mirrored fashion the other way. If the difference is half a mm or so, just install it the same way each time.
Other factors: wheel is crooked in the dropouts. Install on the ground with a little weight through the fork before tightening the skewer for consistency.
If it’s not true, the dishing gauge will not be accurate.
