When installed, the upper bearing sits proud of the frame by about 1mm. Without the centering collar the dust cap will sit flush with the frame. With the centering collar, I get what you see in the picture. Any help appreciated. I'm thinking maybe the centering collar isn't original.

Double check the c-ring,if it works normally and if you don’t have any play otherwise it seems that you are correct in your assumption.
I’d look for another c-ring and some headset shims if needed.
Replaced mine with one for the SL7 and a 0.1mm shim.
Wait for a second opinion on this.
It could also be a large O-ring seal that goes in there – my Orbea had a headset with one.
