Worn chain, worn chainring, or both.
Worn chains tend to stretch out
As has been said before, this is a heavily worn chain. Strongly recommend you buy a chain wear gauge and check your chains from time to time, bearing in mind that a worn chain also wears out rings and cassettes faster than a chain that’s within spec
Replace your chain. It’s stretched.
Most likely you will have to replace whole drive.
New chain won’t marry old cogs.
Just replace it all and take good care of it so it will last many years.
Not 100% sure, but maybe your chain and or chain ring needs replacing
