    Yes, you can see where it should be/was in the first pic, from the chafing.
    Looks like the star nut slid up or was improperly installed as well.

    In any case, you need the stem up to the upper bold. Any other is super dangerous

  3. You’re right. Ideally you should have as little space as possible while still providing a bit of height that can take up the slack after preloading the fork.

    BUT at the very least both bolts should be tightened against the steerer. 

  4. You are right. The steerer tube should not end below the upper screw of the stem.

  5. Yeah, that’s too much – you want about 3mm so there’s enough stem on the steerer but also enough so that the top cap can push down on the stem to load the bearings properly.

    If that spacer is one big thing I’d trim a bit off it – or replace it with individual spacers so it’s more flexible.

  6. Too much spacer, because you can’t make the tube any longer, but you can put less spacer in.

    You want the top of either the top spacer, or the stem, to be about 1mm above the steerer (so it gets clamped down, and not the steerer tube). If the majority of the top bolt isn’t going to be clamping around the steerer, then that’s not safe.
