I want to lift the handle bar a bit to relieve my lower back pain. I am thinking about getting a 45 degree stem. Should I also get a riser stem to lift the handle bar even more? Is there a safety issue with a riser stem?
Don’t do anything that extends that stem above the steerer tube, those sorts of steerer extensions are broadly unsafe. However, there are other stems that provide more rise than a 45 degree stem and are still safe…I’m thinking both the Happy Stem and Cigne Stem by Velo Orange, and I’m sure there are others.

(The Happy Stem is listed as a 45 degree stem but it looks steeper than the one in your picture)
It might be worth it to get a bike fit.
