Remove a link or two so you have more chain wrap around the freewheel.

The tensioner should be at an angle more like this, maybe even more horizontal, wrapping more of the chain around the cog and keeping better tension on the chain. Just remove a couple links.

The cog is very worn. The cog and chain should both be replaced, at the same time.
That should solve the problem of the skipping, if you clean and lubricate your chain.
Chain is too long, remove a link should be good
The tensioner should be pushing the chain toward the teeth of the cog, yours is pulling it away.
Thanks for the advice everyone, will remove a few links and report back once done

More short chain give you maximum tension
Your bearly touching 7 teeth and not enough tension, take 3 links out and just keep experimenting until you get it right
