If it’s noisy, replace it. If not, don’t. They’re cheap.
That’s beyond worn. The outer can be replaced easily enough.

Worn wheel > less friction > more fast. Keep riding it until your chain runs on the metal. Metal on metal > sparks.
Sparks > even more fast.
Not sure if its just a lighting effect, but it appears to be off center. How much bearing/bushing play does it have?
