Bike wheel sometimes shifts and starts rubbing against frame. Am I securing it wrong? I just pull against the frame until the chain is tight and tighten one side, then the other.
I was doing some research and it looks like this is a horizontal dropout. Basically the wheel can sit as far back or forward provided it isn’t too close to the open side? But my issue is it twists and moves as I tighten it. Should there be some spacer somewhere?

Look up a video on how to tighten BMX chain this will require the same procedure.

I assume you’re not tightening it tight enough. It shouldn’t slip – unless you have super-massive quads.

Buy a chain tug and never worry about it again.
Tighten harder
What tool are you using to tighten that nut?
