This is how the authorized distributor set up the hydraulic brakes on my kid's trike. I just noticed from the wear pattern that either the disk is too small or the washers/bracket are wrong. I've never set up post mount calipers and can't figure out what the problem is. Diagrams I've found suggest 2 washers is the minimum. Anybody know what the issue is?

Those are dome/concave spacers not just washers… but you don’t necessarily need them. There is no minimum amount of washers/spacers required, just the correct amount to fit properly. You may want some shorter bolts. The other option of course is larger discs.
looks like the seller swapped the original avid bb7 cable brakes for shimano hydraulics but left the original washers on. just remove them and put them all under the head of the bolt or swap to shorter bolts.
The guy I bought my Catrike from didn’t know the barrel adjuster was stuck and rigged it by just pulling on the cable. It didn’t shift right so I fixed it.
I was looking at a trike from another vendor and he had the nerve to say “I won’t work on it if you buy somewhere else “.
