It’s a workaround for the inherently poor cable routing option on that frame. I’ve worked on bikes since the 90’s and I have never seen this before. The brake calipers themselves look otherwise normal.

So it appears that someone used some canti parts and modified one end to accept the cable ferrule. The pulley looks like another brake part I have on the step through Schwinn High Plains I built for my wife, but modified as well. It looks like it could work but don’t know how well. Those cantis are made to function well with very specific geometry. Seems like more trouble than it’s worth unless you just like to tinker. Which, if that’s your thing cool.
TLDR;
Looks like a mashup of different canti parts. Due to the geometry it may or may not work well.

This unique brake setup literally doesn’t move or work at all, it’s just hooked up to I guess keep the cantis from flopping limply
