What is this lever on the brake cable called and how is it used? Having trouble getting the brakes dialed in on this old road bike

It’s so you can remove a wheel with wide tires. Otherwise, trying to take the wheel off, the tires would get stuck on the brake pads, and you’d have to readjust them. You won’t notice it if your tires are similar width to your rims.
The lever exists so you can flip it up and loosen the brakes, allowing you to remove the wheel. It should be pressed down at all times, basically ignore it unless your tires get stuck when removing the wheel.
