Use a rubber mallot to tap the other side of the crank to ensure it is fully in. Then reattach. Looks fine to me.

  2. No worries, the crank arm bolts come loose with time and the crank becomes loose ,your crank arm probably came off due the the bolts coming loose too just put it back in the correct direction and tighten the bolts and you’re good to go and also make sure the drive side is fully pushed in, it’s probably best to watch a tutorial on YouTube just to be safe and make sure you do it right. and also there’s a cap on the side of the crank arm as a last measure to stop the crank arm for falling off better buy a new one and put it on if it’s lost.

Put it back on! Looks like you’re missing the preload ring that screws into the threaded part of the spindle. The reason it fell off is because the pinch bolts were too loose.

  4. Based on your bro having a very odd type of mechanical failure and a crash, go to your local bike shop (lbs).

This needs to be checked out for a missing/damaged piece, crank arm bolts torqued with a proper torque wrench, and safety checked.

Something fishy is going on if a crank arm comes off. Likely an inexperienced mechanic did the tune up. This could be a first warning of other bolts being incorrectly torqued
