My cousin doesn’t use reddit (loser) so I’m trying to help him out here.
He has a Kickr V5 and and older rim brake Wilier and the chain keeps popping off. I’ve attached a video with his ridiculous commentary but any help will be appreciated.
The bike has a new chain, it’s a new cassette and it only started happening this week.

“Ridiculous commentary”. Lmao. Old mate just sounds like your average bloke from Western Sydney. Most likely a Lebo, or a wog of some kind. The backbone of our country TBH.
As for the bike, you shouldn’t be pedalling backwards, and some bikes will drop a chain if you do. As for the pedalling forwards, it’s not indexed properly. You need to tighten the L screw until the derailleur physically can’t go past that lowest gear.

Index your gears correctly and that won’t happen, unless they have a bent hanger.
The main issue is the high limit screw is too far out. If the limit screw was correct, the chain can’t fall to the outside because the range of the derailleur is limited. Get it? Once the limit is correct, then recheck indexing.
Tighten the high “H” limit screw on the rear derailleur for starters. Are you using a campagnolo cassette and free hub on the kickr or is it the stock one?
At a glance it looks like the whole cassette is sitting a fair way inboard of the dropout – is that the same when the rear wheel is fitted up?
People suggesting adjusting the indexing or the high limit are right in so much that that’s a way to address the overshift on the trainer, but if the cassette positioning on the trainer and the wheel are different then tuning for the trainer just means his setup is gonna be misadjusted when he switches back to the wheel.
Of course, if he only uses this on the trainer then that doesn’t matter, and the best bet is to make sure both the derailluer indexing and limit screws are correctly setup for the trainer.
Another minor correction to some other advice – limit screws don’t control indexing, they simply limit the ends of the derailluers range. Cable tension controls your indexing.
