Got the bike for free. AFAIK everything is stock, so I’m not sure if it’s the BB or the derailleur being off.
Don’t know geared bikes that well, I’m a single speed guy for the most part. How do I fix? It’s a sick KHS frame and I want one geared bike in my fleet.

Is it rubbing? Chainline should be straight in the middle gears not the smallest cog so it’s slightly outrageous but not overly so
It looks to me like your rear wheel isn’t set properly, or, you have a bent crank arm.
You’re bottom bracket is fucked.
It’s essentially bearings between the peddles.
Let’s hope a new button bracket will fix but get your local bike shop to check it hasn’t fucked the frame.

if this is original crankset on the bike, this may be normal as is. chainline is a lil bend like that on smallest cog(and biggest cog too) and the chainring guard (on 1x chainring, to prevent chaindrop) made it ‘look extreme’ while its not.
if its straight on middle cog, you are good.
if you dont like the look of that, you can remove that ring guard, and install chain guide (its installed like front deraileur, though chainline may still look like that)
Put the back in the middle. It’s only going to have a straight chain in one gear.
The chainring is fine, it looks off because the chain is on the smallest cog.
If ever you found the chainline was a bit off, there is room for you to move the cassette out at least 3mm, judging by the gap between chain and chainstay in your pics.
Ok is no one gonna mention that the front chainring doesn’t appear to be parallel with the rest of the frame??
Unless it’s just a weird angle that the photo was taken, you’ll likely need to replace the crank set, unless the bottom bracket has been somehow bent.
One of those two (maybe both??) are the cause of the issue unless it’s the actual frame bent out of shape, then you’re fucked.
Your chain ring should be in-line with the middle gear of the cassette. If it’s off. You need to get a chain ring with a different offset.
