Brought my bike to LBS for a tyre change, was told that my carbon wheel is damaged. Is this caused by improper use of tyre lever by the mechanic or damage from pot holes etc.

I am not a mechanic, but I doubt the tool to unseat the tire would cause that. The tool goes around the rim not through it and is made to not damage the rubber tire so I doubt it would damage a carbon rim.
