  1. In the first photo that screw on top is not tight. If you cannot tighten that screw, it just spins, then the wedge nut on the bottom fell down into the fork. Take the stem out of the fork and tip the bike upside down

  2. You need to take it apart and pull it out. The wedge isn’t in the right place for some reason and no amount of tightening will fix it.

    Just replace the stem with a new quill stem. They’re basic and very cheap. Take it to your local bike shop. They may well have one lying around and if not you’re looking at $20 or so for the part.
