Please excuse the dirt, just got back from a ride.
My gears won’t stay indexed – huge amount of wobble on my back wheel.
Is this fixable? I’m not sure if something can be tightened/ a spacer adding?
Or do I need to look at replacing the hub/wheel?

It depends. It’s a cup and cone hub, so you will be able to adjust the cones to remove the lateral play. The axle could have snapped, but that could be replaced.
If you’ve been riding like that for a while, there’s a good chance that the cones and/or cups are damaged. In that case, the hub will never run smoothly. The cones can be replaced, but if the cups, also known as bearing races, are in bad condition, it’s new wheel time.
