Yes, but you can just change the plastic wheel. It can wear against the tensioner during use and the wall becomes thinner.

Don’t think so. You need a new sproket, but the tensioner looks ok.

Had the same thing happen to me. It happened two or three times that the chain derailed from that little sprocket (which kind of “blocks” the chain), so ultimately changed it, which I’d definitively recommend.
