Also, can anyone eyeball it, my BB shell is 73mm, did it fall off because the BB is 68mm (possibly, i swapped it from another bike without looking)? Also, my cranks were tight for 500+km, then this one fell off.

Crankarm was probably not seated correctly when you tightened it. If it was and you tightened it up nicely then I’m not sure. Saw some people online talking about older revisions of octalink BB’s that can cause this exact problem.
