You have a carbon steerer, which is a hollow tube that can be easily crushed by the the forces of the clamping the stem.
To counter this, under the headset cap is an expanding metal plug which typically is about 30mm deep. This plug pushes out from the inside of the steerer and protects the steerer from getting crushed. Thing is, you’re likely mounting your stem well below that plug, so your steerer can be crushed like a beer can.
There are expandable plugs that are much longer to allow for mounting the stem lower, but I don’t think even those reach that low.
Depending on how much you torqued that stem and how long you’ve ridden it like that, it may already be permanently damaged. Or it may be peachy keen.

It depends on how long your compression plug is.
Ideally your compression plug should be fitted so that it braces the steerer tube the full length of where your stem bolts sit. In practice – many people run compression plugs that don’t brace the lower bolt – but the closer to the lower bolt the better.
If your compression plug doesn’t reach the lower bolt – then be extra careful with your torque.

That depend entirely on the stem. The reason you wouldn’t want to do this is that the internal plug doesn’t go down deep enough to reinforce the tube opposite of where you tighten the clamp bolts. So, you could run it like this, but you need a longer plug that properly reinforces (pushes out) from the inside in the appropriate location.
You have to think of all the parts together, the steerer, the expanding plug, and the stem clamp and consider how they all interact together.
So yeah, I’d cut the tube down if you want to run the stem that low.. or try to find the appropriate expanding plug that will expand in the desired location.

Just hammer in like 4 star nuts using an old headset bolt and an adjustable wrench, youll be fine.
