Worn out cassette. It’s time to change that as well.
Usually it’s best to change the cassette and the chain at the same time. Assuming the cassette is okay, tho…
Have you adjusted any of the limit screws or cable tension? After changing components in the drivetrain you will usually need to adjust those.
Are you saying this is happening after adjustment? If not, adjust it and check back in.
