Is your hangar bent? Looks a bit bent by eye but that might be angle/lighting
Your derailleur hanger (or the integrated one that’s part of the frame) looks pretty bent
How did you check? That hanger is visibly bent!
I’ve looked at both photos and looks bent imo. With an integrated hangar this is a really quick check and fix at a bike shop.

Aside from possible hammer alignment, what components are you using? Are you exceeding the capacity of the derailleur? For example, if you’re using a rear derailleur with a max sprocket size of 46 with an 11-50 cassette, this is one of the issues that could be expected.
In addition to checking if the hanger is bent Adjust the b tension
