At least 3 threads should grip in order to be safe. If its less, than you need a longer axle, IMO

No. Minimum thread engagement in steel is 1 – 1.5 times the bolt diameter while in your pic you have ~ .5 or less, and that is assuming 100% engagement which never happens in real life; 60% – 80% range is most common.

You can get a nut without the integrated washer and put on a thinner washer. It should help get more threads in.
