I’m having some trouble removing this old non sealed bottom bracket. There doesn’t seem to be teeth for using a usual sealed cartridge removal tool. Or did it used to be sealed but is broken? I’ve managed to take one side out, but there’s still bearings stuck on the other side along with a casing in the middle.
I’ve tried using an adjustable wrench around those two flat edges but no dice. It just slips off. My vice grips aren’t wide enough. Attempting a 90s mtb conversion so will replace with a new bb.
Any ideas?

If you’ve got access to a bench vise, use it to grip the flats of the fixed bb cup and then rotate the frame (this provides additional leverage). Probably a good idea to apply Liquid Wrench or another penetration oil 24+ hours in advance.

This is the fixed cup on a cup and cone bottom bracket. You probably need a 36mm wrench.

Fixed cups can be tricky to remove. They are reverse threaded and after decades they can be very very stuck.

Or take it to your local bike shop. Mine charged me about $15, despite a lot of grunting and cursing, although some may charge more.
