The picture on the package shows the original intended use.
Someone got tired of using frame mounted shifter and replaced them with mtb-/or flatbar shifter.
It looks like it’s just there to hold the shifter. Where does the shifter cable lead to? It could operate a parking brake, but I don’t see why you’d need one, or possibly a secondary gear system like a hub gear, both the normal gear cables seem to be there.

Can you show a pic of where the cable goes?
Does it tie to the shift cable from the other shifter?
It almost looks like some kind of old school climbing shifter.
When I saw how this was being used in that picture, I thought, “That looks like just the sort of thing that Problem Solvers would address” – and they do, sort of – [Handlebar Accessory Mount](https://problemsolversbike.com/products/handlebar-accessory-mount).
I note that it’s not made to work directly on a 22.2 mm bar.
Could be an accommodation for a disabled rider?
Without seeing where the cable goes this is just conjecture, but it could be holding the shifter for a 3 speed cassette freehub. I have one for my touring configuration. It’s nice to have 60 gear options ranging from “grinding at 30mph” to “spinning out while climbing a 20% grade fully loaded”
