its a friction fit plastic shell, just pull it up and a threaded headset will be under it.
it will be easier if you pull the quill stem out, then the plastic cover/shell will pull right off.
For this type of bike, many of those caps have wrench flats and are the top cap of the threaded (often zero-stack) headset. It looks and sounds like that’s not the case, and usually it’s just a semi-sealed cap with grommet to fit snugly against the bar stem.
It may sound obvious, but have you already tried sliding/wiggling it up off of the headset?
Moreover, if for some reason it’s difficult to slide it off (assuming it’s what I think), you could loosen and remove the bar stem to reveal how and to what the cap is attached.
as others have said already, you need to take off the stem first. This kind from Gazelle is a quill stem, there is probably a stem bolt covering the hex bolt. you will take a long hex key, like 6mm/8mm. Once it’s off, you can pry off the dust cap and get access. the headset will be threaded and contain two opposed nut. I would be very surprised if that’s still loose ball and cup though.
