I remember the dust seal covers the bearings but I don’t remember how these other 2 washers/spacers are supposed to be arranged. Also a good chance I’ve lost the other washer you see on the end here. Are the nipples supposed to press into the frame followed by the outer washer and axel bolt, or is the grippy side of the washer supposed to interface the frame?

Hard to tell but it looks like you’re missing the locknut for the bearing cone. Without this it would be fairly easy to undo the cone (what you have described as the bearing cover)
