    Could be that it has come loose, but is a bit unlikely. Under normal use, each pedal stroke on the bike should be tightening the free hub down more and more. They tend to be on there really well by the time they get worn, but I’ve seen crazier things.

    That being said, my suspicion is that there is an issue with either the ratchet or the bearings in this free hub. I’d recommend taking the wheel into a shop and asking them to torque it to spec, a good shop will do this for free. If that doesn’t address the problem, it will need to be replaced, which should come out to ~$40 for parts and labor depending on your market.
