SRAM gx eagle derailleur and shifter on a new to my 2018 Specialized Epic comp. Had chain issues from day I got it.
So far I’ve:
– replaced the chain
– tensioned the derailleur
– set b screw using sram adjuster tool
– set lower and upper limit
– checked pulleys for tightness
– visually inspected hanger.
Done all of the above a few times now. Still skipping in middle cogs. Sounds like it’s rubbing periodically on the cast and cases gears to hesitate and skip periodically under pedaling. Last thing I can think of is buy a new wolf tooth metal hanger or perhaps cassette has bend cogs in the middle due to previous owner crash.

Bent derailleur hanger.
Had similar symptoms (rubbing and all) recently and a new derailleur was the cure. Off the bike I see no signs of failure, but I knew I had beaten it into rocks a few times over. I can see similar wear on your derailleur.
I even thought about doing a hanger and new shifter/cable housing but I didn’t have those parts readily available. I just replaced the derailleur after battling it for a few weeks of skipping under load and its smooth as butter now.
